Our practice follows the guidelines for immunizations and routine lab tests recommended by the American Academy of Pediatrics.

In fairness to all patients, we ask that you be on time for scheduled appointments. Please, do not walk in without an appointment outside of our walk-in hours. Every effort will be made to accommodate patients who arrive more than 30 minutes late or walk-in without an appointment, however, in order to maintain the flow of patients with appointments, your child may have to wait in the waiting room longer than if an appointment had been made or kept. A fee of $25.00 may be charged for visits without an appointment outside of our walk-in hours.

If an abnormality is encountered or a pre-existing problem is addressed during a routine preventative well visit, an additional diagnostic code may be needed. Your insurance company may apply a co-payment or deductible for this portion of the visit. Please check with your insurance company regarding your coverage for these services.

Co-Payment is due at the time of the visit. Self-pay patients are expected to pay in full at the time of service. Please check with your insurance company regarding your coverage for these services.

All forms required by our office are found on our website or may be picked up at our office. Prescription refill requests are best handled at the time of your child’s visit. If a prescription is needed at another time, (except for Schedule II prescriptions), it should be called in directly to your pharmacy, so they may send us the refill information electronically. Referral request forms require 5 business days for completion. Except for emergency room visits, referrals cannot be back dated.

Patients needing behavioral disorder medications (ADHD, anxiety, mood disorders) must be seen every month until the proper medication and dose have been established. Thereafter, patients should be seen at least every 3 months and medications may be prescribed for up to 90 days at a time. Patients also need to be seen if there is a change of dose or a change of medication. ADHD and mental health prescriptions require a written request and a minimum notice of 5 business days. Outside of a visit, the prescriptions must be picked up and a fee of $20.00 will be due.

One health/camp/sports form will be completed at the patient’s annual well visit at no charge. A processing fee of $15.00 will be charged for each additional form completed. The patient’s section should be completed before forms are dropped off. Please allow 7 business days for completion. Upon your request, and if available, forms may be expedited for an additional fee.

In order to cover the laboratory operating costs and maintain our high quality of care, a convenience fee of $25.00 is charged for all specimens other than rapid strep and quick urine tests. Should you wish not to pay this fee, you may go to the laboratory that contracts with your insurance company. Please be aware that although your child’s specimen is collected at our office, it is sent to Quest laboratories for testing and Quest will bill your insurance for the tests.

Test results and records for patients 18 years and older will not be released to anyone other than the patient without a signed release. For transfer of records, a signed release and a processing fee of $30.00 is required per record. Please allow 7 business days for completion.
