The below forms may be downloaded and either brought into our office or faxed directly to our office at (703) 532-8426.  Please fill out all forms completely.  One of our staff members will call when the form is complete and ready for pick up.  A $10.00 processing fee will be charged for all school /camp /sports forms.  All processing fees will be due at time of pick up.

Please check all forms to confirm the date last physical is required. Patient’s last physical may satisfy requirements and a new physical may not be necessary. Verify and understand your coverage as some insurance companies may not cover a second physical within the same year.

So that we may best accommodate our patients, we ask that you call us at least six weeks before your child needs to be seen for School, Sports and Camp physicals. June, July and August are our busiest months for physicals. The earlier you can call to schedule these appointments, the better the selection of times you will have to choose from.
